3. Availability

I am available to schedule counseling sessions Monday through Friday. I have limited evening availability for later counseling sessions (typically 5pm is my last scheduled session). However if your needs fall outside of this availability, please let me know and I will try to accommodate you if I am able.

4. Confidentiality and Records

You as the client have a right to confidentiality. There are exceptions to confidentiality when you provide me with written permission in the form of a release of information to disclose identified information to a third party. 5. Referrals

If at any time you are dissatisfied with my counseling services, please let me know. I will do my best to work with you and resolve your concerns. If I am not able to resolve your concerns, I will provide you with a referral for counseling services requested.

6. Emergencies

I check my voice mail multiple times per day during the weekdays. I will do my best to return your call within 24 hours. I am not however, able to respond to a mental health or counseling emergency situation. If you experience an emergency, please contact 911 or go to your nearest Emergency Room. 7. Complaints

If you feel my actions in the counseling session are inappropriate, I request that you bring it to my immediate attention.
